As parent/guardian of the above named child I give permission for them to audition for the pantomime.

I understand that all decisions are made by Little Wolf Entertainment and are final.

I give permission for my child to appear in promotional photos and video taken at the auditions.

My child will be at least 9 years old when rehearsals begin.

My child will NOT have left year 11 at school when the pantomime takes place.
